  • Municipal Tax Bill Copy with payment receipt-Home of Applicant (3 copies)
  • Rent agreement of shop if the property is taken on rent with latest electricity bill (3 copies)
  • Copy of ownership document/Index copy of shop if property is owned by applicant (3 copies)
  • Electricity Bill Copy-Shop (3 copies)
  • PAN Card Copy of applicant- (3 copies)
  • Aadhar card/Election card/Driving License/Passport copy of applicant -(3 copies)
  • 3 color passport size photos of applicant
  • Name and type of business of Business
  • Date of start of business
  • Number of people working under applicant
  • If Gumastadhara applied on family property; then an affidavit on stamp paper of Rs. 20/- required ( 3 copies)
  • If shop or property has not been registered or transferred in municipal tax bill; then an application with receipt of fee paid required ( 3 copies)
  • If business is under partnership / LLP / trust form, then copy of partnership deed ( 3 copies)
  • If business is under company / society form, then copy of Memorandum Of Association ( 3 copies)
  • Copy of any other government registration or license ( 3 copies)
